The winner will be selected in a random drawing on December 29th, 2008, and the winner will be published on achooallergy.com on the same day. The winner will be notified via email, telephone, or certified mail. Potential winners will have five days from the date of said notification to respond to such notification. Potential winners must sign an Affidavit of Eligibility and Liability and Publicity Release before claiming the prize. Failure to comply with the official rules, failure to respond to notification within five days, or failure to sign required forms will result in disqualification, at which time another winner will be selected at random.
